Let's say you complete forms on websites accepting link requests. Under the comments section, do not try to sell the website on trading links and how it's good for google PR, blah, blah, etc. If the website encourages link exchanges in a form, they already know the importance of links. You bore the reviewer. This causes them to delete your link application.

If the form asks for your website title and description, examine existing links on the website. Do this before writing your own title and description. Suppose they are:

short capitalized titles,
with one sentence descriptions,
which start with a capitalized letter, and
end in a period.

DO NOT write a 10 line description, and title, in ALL CAPS ending with 5 exclamation points. The reviewer has to edit your information if your website is approved for a listing.
If the reviewer does not want to rewrite your website title and description, they simply delete your link application.
