Human Incubator Kangaroo care is a cost-effective concept that helps nurture premature infants to health
These days, the most important part of Nilima Gimbhal’s daily routine is to wrap her month-old baby boy in a cloth bag and hold him close to heart.
Born in the seventh month of the pregnancy in April, the premature baby weighted only 1.24 kg at birth. A normal new-born has to weights more than 2.5kg.
Gimbhal’s act of binding the baby to her chest for 14 hours a day is inspired by the female kangaroo, which carries its joey in a skin pouch in its belly.
Called Kangaroo Mother Care, the concept is gaining popularity in the city as a means to nuture premature babies to health. Last month, SL Raheja Hospital in Mahim introduced the maternity and child care technique for its patients.
Underweight premature babies are conventionally kept in incubators, which provide warmth and insulated the baby from possible infections. Even mothers are denied access to the baby.
“With kangaroo care we want to promote an ambience where the mother and child are encouraged to bond,” said Dr Uma Nambiar, chief executive officer, SL Raheja Hospital.
The concept involves skin to skin contact and is based on the fact that warmth of the mother’s body helps boost development of a premature baby.
The ‘human incubator’ allows transfer of body warmth thus helping in faster weight gain and recovery of the infant. “The baby senses the mother’s heartbeat and breathing and this regularizes the baby’s breathing,” said Dr Asmita Advirkar, associate consultant, neonatology, SL Raheja Hospital.
Kangaroo care also promotes emotional bonding between mother and child. “Any healthy family member can provide kangaroo care. It is basically about human warmth, “said Advirkar.
While private hospitals such as SL Raheja are new to advocating the important of kangaroo care, KEM Hospital in Parel, a public hospital introduced the concept in 2000.
KEM’s Kangaroo Mother Care Centre nurses approximately 1,000 babies every year. The centre monitors the health of premature babies referred to it and trains parents to administer kangaroo care.
Chandrashekhar Pawar, 32, an electrician, helps his wife Sujata in giving kangaroo care to their two-month old boy. Born three month’s premature, the baby developed respiratory and cardiac distress and was put on kangaroo care 12 days after birth.
“We have been asked to give kangaroo care till his weight reaches 2kg. It is a better and ad cost-effective technique than keeping our child in an incubator,” said Pawar, a Charkop resident. “His increasing weight is our biggest motivator: Also, we can do this anytime, anywhere.”
“Kangaroo care is a cost effective and comprehensive method of care for premature babies with low weight. Early, continuous and prolonged skin-to-skin contact facilitates breast feeding provides warmth to the baby and prevents infections,” said Dr Ruchi Nanavati, professor and head, department of neonatology, KEM Hospital.
“We have been giving kangaroo care to all the babies weighting less than 2kg at birth, “said Nanavati.
Nanavati oversees the centre where Gimbhal, 21, learnt how to administer kangaroo care to her first-born. After a month of following instructions, Gimbhal’s son, who suffered from a breathing difficulty and other complications, has gained approximately 300gm. “My child was so tiny my husband and I were afraid to hold him, “ said the Borivli housewife. “ I want my baby to be healthy. Holding him every day I have noticed that he is gaining weight.”

Liver care tips for a healthy Liver

Obesity now seems to be the main trigger factor for liver diseases, as doctors in the city say that with a sedentary lifestyle obesity is on the rise and that is the main cause of liver disease.

With liver transplants bringing in a bill of between Rs. 12 to Rs. 30 lakh, on World Liver Day doctors emphasize the need for nipping the problem in the bud so that liver diseases can be prevented.

With the increase of obesity there seems to be a silent epidemic of non-alcoholic fatty liver problem, which gradually leads to severe liver damage.

With changing dietary habits and a sedentary lifestyle, more obese people are showing up with fatty liver disease.

Since the cost is so high when it comes to liver transplants, it is of utmost importance that the cause is tackled at the root. Most of the causes which lead to end stage liver disease like alcoholism, obesity or hepatitis are controllable factors. So it is essential that people are made aware of the triggers and how to prevent them.

The increasing problem of Nonalcoholic fatty liver disease (NAFLD) occurs because of diversion of excess calories to synthesise fat in the liver and the inability of the liver to export the synthesised fat.

“Insulin resistance is the primary pathophysiology of this disorder. Excess fat in the liver causes inflammation, a stage called Non Alcoholic steatohepatitis (NASH) which is seen in around 25 per cent of NAFLD patients. Over time this inflammation causes progressive liver damage leading to cirrhosis and liver failure,” said Dr. Poorna Chandra, Consultant Gastroenterologist, Fortis Hospitals.

The pricy transplant procedures and the expenses incurred for being on immunosuppressant drugs for one’s whole life are so high, that doctors insist on the need to tackle the problem at its onset, so that it does not graduate to end-stage liver disease.

Some of the Common Liver Related Diseases

1. Hepatitis: inflammation of the liver, caused mainly by various viruses but also by some poisons
2. Non-alcoholic fatty liver disease: A spectrum in disease, associated with obesity and characterized as an abundance of fat in the liver
3. Cirrhosis of the Liver: This is the formation of fibrous tissue in the liver from replacing dead liver cells. Diagnosis is done by checking levels of Alanine transaminase and Asparatine transaminase (SGOT).
4. Haemochromatosis: A hereditary disease causing the accumulation of iron in the body, eventually leading to liver damage.
5. Cancer of the liver: Primary hepatocellular carcinoma or cholangiocarcinoma and metastatic cancers, usually from other parts of the gastrointestinal tract.
6. Wilson’s disease: A hereditary disease which causes the body to retain copper.
7. Budd-Chiari syndrome: Obstruction of the hepatic vein.
8. Gilbert’s syndrome: A genetic disorder of bilirubin metabolism, found in about 5% of the population

Virtual knee surgery in Mumbai

This may seem like something out of a movie, but virtual surgery has just made a mark in Mumbai. 61-year-old Neena Aggarwal is patient who has started walking – after a decade- thanks to this latest advancement. Neena suffered with severe osteoarthritis which resulted in bow shaped legs, and the fact that she weighs 114 kilos, made surgery a risky option.

“Every possible balm she has applied, every possible medication she has tried and it was torture. It has actually been torture for her,” said Neena’s daughter.

Doctors decided to for virtual treatment – performing what’s known as Patient Specific Instrumentation or PSI Knee Replacement surgery – for the first time ever in Mumbai.

What happens during the PSI knee replacement surgery is that the doctor prepares an exact 3D image of the knee on the computer – and that replica helps the doctor to virtually plan for the entire surgery – so that the surgeon knows, what cuts to make, what the alignment should be like – so much so, that the implant sits perfectly on the knee (fitting the model-implant).

This surgery has only been performed in the US, Canada and England, and there are clear benefits, doctors say.

Dr Kaushal Malhan, Knee and Joint Replacement Surgeon, Fortis Hospital says, “Obviously when you are taking lesser time, when you are doing less damage to the tissues , when you are cutting less, when your planning is already done and you already know what you are going to do in theatre, in terms of what angle and placement, I think you are on surer footing, in terms of that, so probably the complications may be lesser.”

Considering one in eight knee replacements in India is unsuccessful, according to the Arthritis Foundation of India, that’s the best news for Neena, even though this sort of surgery costs about Rs 40, 000 more than traditional surgery.

Neena’s daughter says that her not just her mother, but her father is also looking forward to the surgery anxiously and has already planned a post-surgery vacation.

“My dad has already planned a trip with her he’s like, once you are out of the hospital, we are planning a trip to Goa,” said Neena’s daughter.

Ischemic Heart Diseases On The Rise in Karnataka

Ischemic heart disease (IHD) is the No. 1 killer in Karnataka, accounting for every 12th death in the state.

The disease is caused by reduced blood flow to the heart; some 43.12% of deaths are caused due to IHD. The 2009 report on medical certification of cause of death (MCCD), prepared by the office of the chief registrar of births and deaths, shows that every 10th reported medically certified death has been that of an infant (less than one year of age).

Bangalore tops the list with nearly 30% of over 1.15 lakh registered deaths in the state. Medical certification of cause of death provides data on death by age and sex in respect of specific causes. Besides IHD, which is categorized under ‘diseases of circulatory system’, deaths due to “certain infectious and parasitic diseases” is the next most killer. Tuberculosis and septicemia constitute 43% and 21% of deaths under this major group of diseases.

Poisoning by drugs and biological substances and toxic effects substances, which is categorized under deaths due to “injury, poisoning and certain other consequences of external causes”, is the third most killer in the state. The government obtained information from over 1,200 hospitals. At least 85% of infant deaths have been reported due to certain “conditions originating in the perinatal period”. Among the children aged between 1 and 4 years, “certain infectious and parasitic disease” has taken the highest toll of 32.03%. When taken gender-wise deaths, more than 63% constitute male.

What is IHD ?

Ischemic heart disease is caused due to decreased blood supply to the heart. It happens due to blockages developed in the blood vessels. These blockages are cholesterol deposits along with calcium; 99% of heart diseases are due to these blockages, says Dr Deepak Krishnamurthy, cardiologist, Fortis Hospitals.

My wife Mrs. Saswati Deb Sikdar was admitted in the Emergency department of the Fortis Hospital, Anandpur on 4-4-11 in a critical condition. She was diagnosed “Urosepsis with septic shock and multi organ failure”. She developed episode of hypoxia and bradycardia . She was immediately admitted in the ICU of the hospital and managed with the mechanical ventrilator, Vesopressors and haemodylasis. While admitted in the hospital she also developed fungal skin infection and resistant candida from sputum. This was also treated for the same with topical and I/V antifungal. She gradually recovered and finally released from the hospital on 16-4-11. Still she is reporting to Dr. A.R. Dutta and Dr. K.K. Gangopadhyay in the outdoor department of the hospital and recuperating.

I shall be failing in my duty if I do not mention the care and sincerity she received from the attending Doctors, nursing sisters, paramedics including physiotherapist and housekeeping staffs. My special thanks are due to (to name a few) Dr. Yashesh Paliwal, Head of the Critical Care and his team of doctors in the Emergency department together with Dr. Arup Ratan Dutta, Chief Nephrologist and Dr. Sumit Mukherjee. I believe that their sincerity and timely intervention as a professional could save the life of the patient. Her recovery in the hospital is a wonder to all those who went to see her in unconscious state at the hospital. My family joins me in saying a big “Thank you Doctors!”.

7th Time Hip Replacement on a Iraqi Patient at Fortis Hospital

Setting yet another milestone in orthopedic surgery, Dr Sachin Bhonsle at Fortis Hospital Mulund performed a complicated hip replacement revision surgery on a 52 year old Iraqi lady who had undergone 6 failed hip surgeries before.

Dr Sachin Bhonsle, Consultant Orthopedic Surgeon took up a challenge to perform the 6th followed by 7th revision putting the patient back on her feet. Sixth is a preparatory for the final revision (7th surgery).

Patient Ms Nadia, a teacher from Baghdad had a childhood injury to her right hip which damaged the joint progressively till it developed arthritis. When she was 40 years of age she decided to opt for a total hip replacement which was performed in her own country. The surgery did not yield good results and her joint soon dislocated. She had to undergo second surgery to revise the socket. This surgery gave her benefit but only for two years.

In 2002 the hip developed problems once again and she had to have a revision surgery for the third time. This failed again after three years.

In 2005 a fourth surgery was performed to change the entire hip joint. She got a good pain relief after this surgery. This hip joint had loosened out by 2010 when a fifth surgery was performed to change the joint surfaces. She did not get any benefit from this surgery.

Reasons for failure:-
1. Unstable joint replacement and poor soft tissue balance
2. Early wear and tear because off poor alignment
3. Loosening because of inadequate fixation in bone and suboptimal cementing
4. Extensive soft tissue scarring and inexistent muscles because of repeated operation in same place

Meanwhile destiny gave her a sixth chance, having suffered for so long Ms Nadia mustered the courage to rectify her problem and decided to come down to India and get operated upon her hip once again.

Dr Sachin Bhonsle says “This case is a fine example of a very complicated hip replacement operation as Nadia had undergone several hip replacements before and those operations were failures. Bone had become brittle due to intense scarring and there was a major possibility of fracture. Nerve damage was possible during approaching such hips because of nerve entrapment in severe scarring. Her poorly controlled diabetes proved a further challenge.”

Dr Bhonsle had to first perform the surgery (6th) to remove all the implants, bone cement and clean up the infection. He performed the final revision surgery (7th surgery) during 1st week of July.

“I opened up the wound and thoroughly cleaned it again. I first prepared her socket (used a cement less socket of a modern design which can last for many years) and put some bone grafts in the depth. Then i went on to prepare the femur which proved a huge challenge. There were many holes and breaks in the femur. I had to use a titanium stem of great length to span breaks in the bone. In addition i had to strengthen the bone using- bone grafts, titanium mesh, plates and screws. It took almost 7 hours to complete the procedure with satisfaction.” adds Dr Bhonsle.

The surgery was successful with an uneventful recovery and Nadia was able to walk within 48 hours of surgery. Dr. Bhonsle is very optimistic that the patient will lead a normal life and will not require another revision in her lifetime.

“Having undergone 5 surgeries before, doctors abroad told me that a sixth correction could never be performed on my hip. I thought that I will have to lead a challenged life for the rest of the years. But like the saying goes every cloud has a silver lining! I too got hope after 12 years of suffering from Dr Bhonsle who took up the challenge and cured me. Go under the knife to lose weight and cure diabetes


The best management of type 2 diabetes in obese people is not elusive anymore. Doctors in the city have successfully treated diabetes in obese individuals by wielding their scalpel and removing excess fat of the patients.

Sixty-five-year-old Vijayalakshmi was suffering from hypertension and diabetes mellitus for over 10 years and bronchial asthma for 15 years. Her blood sugar levels were fluctuating despite being administered oral hypoglycemic and insulin injections. Poor control of hypertension also threatened to worsen the situation. Morbidly obese, she had a body mass index (BMI) of 41.2 as against a normal adult human body’s BMI of 20-25.

A laparoscopic bypass procedure with Roux-en-Y Anastomosis was performed on her under general anesthesia at Fortis Hospital.3 months after the surgery, she has now lost 25kgs,her BMI has come down to 32 and her blood sugar is completely normal. She does not take any medication for hypertension, but has only one oral hypoglycemic every day.

Talking about the treatment of diabetes and obesity at press conference on Thursday, Dr Shabeer Ahmed, consultant surgical gastroenterologist and minimal access surgeon, said, ‘‘The continuing mortality in person with diabetes is a sign that the answer to best management for type 2 diabetes for maximizing metabolic control is still elusive. Given this scenario, the option of stomach intervention needs to be considered in appropriately selected individuals. Laparoscopic gastric bypass procedure was considered as the treatment of choice for her as she had obesity with diabetes mellitus.”

It has been estimated that the risk of developing Type2 diabetes increase 93-fold in women and 42-fold in men who are severely obese rather than of healthy weight, Dr Ahmed pointed out.

43 year old Reshmi Begum, who was suffering from diabetic mellitus for 15 years, approached the hospital in April. She also had hypertension and ischemic hear disease for which she had under gone coronary artery bypass graft in 2010.During her evaluation process it was found that her blood sugar levels varied between 25-493 for which she was taking 70 units of insulin daily. She was obese and her BMI was 45(Height 155cms/weight 110kg).
After consolation, she was advised to undergo laparoscopic gastric bypass surgery to address her diabetes and obesity. Her diabetes is now completely under control and she has lost about 32kg of weight.



An Unusual Case of Memory Decline

New variant Creutzfeldt-Jakob disease (nvCJD) is an acquired prion disease causally related to bovine spongiform encephalopathy that has occurred predominantly in young adults. All clinical cases studied have been methionine homozygotes at codon 129 of the prion protein gene (PRNP) with distinctive neuropathological findings and molecular strain type (PrP (Sc) type 4). Modeling studies in transgenic mice suggest that other PRNP genotypes will also be susceptible to infection with bovine spongiform encephalopathy prions but may develop distinctive phenotypes. New variant Creutzfeldt–Jakob disease (vCJD) was first identified in the UK in 1996, and was causally linked to bovine spongiform encephalopathy. Initially misnamed as ‘mad-cow disease’ it went as a taboo with beef eaters. However it has been identified as a prion disease with dissimilarities with classical CJD both clinically and radiologically throughout the world. Herein we report a case of possible nvCJD from India.

A 64-year-old man, Islam by faith, who had lived throughout his life at Calcutta, India. Presented with sub acute onset and progressive depression, irritability, personality change. He was first assessed by psychiatrist and antidepressants failed to arrest the course of illness. The clinical features extended to cause gait ataxia and cognitive impairment which made him seek neurological consultation. His initial MRI of the brain revealed minimal signal alterations around the thalami and he had a normal EEG. Routine blood reports were normal except marginally raised liver enzymes. However he continued to deteriorate and by 3 months he needed support for his Instrumental activities of daily living. She has by then developed myoclonic jerks in generalized fashion. This time EEG did show the typical appearance of of periodic complexes with a much slower background. His CSF study showed raised protein and normal sugar with mild Lymphocytic pleocytosis. However PCR for Tb was negative and fungal smear was also negative. Brain magnetic resonance imaging this time revealed high signal lesions involving bilateral caudate nuclei, left lentiform nucleus, bilateral dorsomedial thalami and pulvinar on fluid attenuation inversion recovery, T2- and diffusion-weighted imaging. The patient developed a kinetic mutism at 4 months and now after 6 months remains vegetative, completely bedbound. Presently she remains on Clonazepam, Sodium valproate and supportive care.

The clinical presentation and neuroimaging findings were compatible with the nvCJD cases reported since 1996 for probable nvCJD. A human disease thought due to the same infectious agent as bovine spongiform encephalopathy (BSE), or mad cow disease. Both the human and bovine disorders are invariably fatal brain diseases with unusually long incubation periods measured in years, and are caused by an unconventional transmissible agent, a prion, resulting in the deposition of amyloid tissue that causes a breakdown of brain tissue leaving the infected brain with a “spongy” (“spongiform”) appearance. The disease in humans is sometimes called variant Creutzfeldt-Jakob disease (nvCJD). The BSE agent has been identified in the brain, spinal cord, retina, dorsal root ganglia (nervous tissue located near the backbone), and the bone marrow of cattle experimentally infected with this agent by the oral route. In addition to cattle, sheep are susceptible to experimental infection with the BSE agent by the oral route. Thus, in countries where flocks of sheep and goats may have been exposed to the BSE agent through contaminated feed, there exists a theoretical risk that these animals may have developed infections caused by the BSE agent and that these infections have been maintained in the flocks, even in the absence of continued exposure to contaminated feed (for example, through maternal transmission). Everyone with nvCJD appears to have eaten beef and beef products at some time during their lives (but then so have most of the population). Investigation of a cluster of cases with nvCJD disclosed that most of the people were likely infected through their diet. Beef carcass meat appears to have been contaminated with the BSE agent in butchers’ shops where cattle heads were split.
